Part II. Research, Teaching, and Clinical Contributions:
A. Narrative Report of Research, Teaching, and Clinical Contributions
As Director of MRI at Beth Israel Deaconess Medical Center for 12 years,
I recruited and led an interdisciplinary MRI research group that has become
internationally recognized for its pioneering contributions in the field
of Magnetic Resonance Imaging. The Group was the first to demonstrate
the clinical feasibility of coronary artery imaging with MR angiography,
to apply perfusion and diffusion MRI to the early diagnosis and characterization
of stroke, and to perform anatomic and functional MRI of the lungs. We
developed and/or implemented numerous techniques for MR imaging and angiography
that are now in widespread clinical use, including segmented turboFLASH,
magnetization transfer MR angiography of the circle of Willis, and double
inversion black blood imaging. I contributed in several respects: as Director
of the research group, as an inventor of new techniques (as evidenced
by multiple patents and scientific publications in the field of MRI),
as coordinator of clinical research studies, and as mentor of other researchers
and clinicians within the Group.
In addition, our MRI Division was nationally recognized for its clinical
strengths, with particular clinical expertise in the areas of cardiovascular
disease and abdominal imaging. We published widely cited clinical papers
including ones concerning MR angiography of the extracranial carotid arteries
and abdominal circulations, detection of stroke with perfusion and diffusion
imaging, and the detection and characterization of hepatic metastases.
In addition to administering the clinical activities of the MRI Division,
my teaching activities were extensive, including daily clinical readout
sessions, didactic lectures to the residents, fellows, and staff, participation
in numerous external courses of multiple national and international societies
including RSNA, ARRS, ISMRM, SCBT/MR, AHA, International MR Angio Club,
and CMRS. In addition, I organized and ran the 1999 Harvard MRI course
in Kauai.
As Chairman of Radiology at Evanston Northwestern Healthcare since July
2000, I oversee a full service academic department with 40 radiologists,
three hospitals, outpatient clinics, procedure volume of between 350,000
- 400,000 annually, an approximately 6000 square foot small bore NMR research
facility, and a radiology research group of seven. I am principal investigator
for a whole body MRI research collaboration between ENH and General Electric
Medical Systems involving 1.5T and 3.0T research systems and a collaboration
with GEMS involving 16-slice multi-detector CT for cardiac applications.
I am also principal investigator on two R01 grants from the National Institutes
of Health focusing on new MRI technologies for cardiac and angiographic
imaging.

The purpose of this study is to enhance the quality of magnetic resonance
angiography (MRA) using 3 tesla to the point that it can replace catheter-based
digital subtraction angiography (DSA) for evaluation of renal and peripheral
vascular disease. The proposal will improve diagnostic accuracy and greatly
reduce the frequency of superfluous and potentially hazardous invasive
procedures, thereby advancing patient health.
“A DIFFUSION TENSOR IMAGING STUDY OF HIV-DEMENTIA”

“BREATH-HOLD 3D MRA OF THE CORONARY ARTERIES USING PARTIALLY PARALLEL
DATA ACQUISITION”

The major goal of this project is to implement breath-hold 3D MRA, first-pass
contrast enhancement, and partially parallel data acquisition, which will
provide unprecedented flexibility and robustness for coronary artery MRA.
To optimize the enhancement of the coronary arteries using contrast agents
to maximize the signal difference between the coronary arteries and adjacent
fat and myocardium as will as signal-to-noise of the coronary arteries.
The key is to optimize the MRA method in patients with significant coronary
artery disease, and to determine if the use of contrast agents provides
any clinical benefits.
“SMASH CONTRAST-ENHANCED 3D MR ANGIOGRAPHY”

The specific aims of the proposal are: 1) To develop multicoil arrays
suitable for imaging of the extracranial carotid and renal arteries; 2)
To evaluate SMASH CE-3DMRA using an ameroid-occluder pig model of renal
artery stenosis; 3) In patients, using x-ray angiography as the correlative
gold standard: a. To determine if SMASH, by improving spatial resolution
of CE-3DMRA of the extracranial carotid arteries, enhances the accuracy
of measurements of stenoses compared with conventional CE-3DMRA, b. To
determine if SMASH can be used to reduce the scan time for CE-3DMRA of
the renal arteries.
“MR IMAGING AND QUANTIFICATION OF PULMONARY PERFUSION”

The aims of this project were: 1) To develop the methodology to a. image
and quantify pulmonary perfusion using first-pass imaging after administration
of a bolus of a gadolinium chelate in a pig model and humans, b. minimize
breath-hold times for MRA by combining a minimal TR, partial Fourier reconstruction,
and sinc interpolation along the 3D partitions direction, c. measure breath-hold
capabilities for healthy subjects and patients with pulmonary embolism
using navigator techniques; 2) To determine the sensitivity and specificity
of MRP and MRA for the diagnosis of pulmonary embolism in a pig model;
3) To perform a preliminary study of MRP and MRA in patients with suspected
pulmonary embolism.
“MAGNETIC RESONANCE IN RENOVASCULAR HYPERTENSION”

The specific goals of this project were 1) to optimize the imaging parameters
for 2D star, and breath-hold contrast-enhanced 3D and multisection 2D
MRA in a pulsitile flow phantom and in normal volunteers. Additionally,
both MRA and CTA techniques will be optimized in a porcine model of renal
artery stenosis; 2) to test the hypothesis that MRA and CTA accurately
depicts normal renal anatomy, including accessory vessels; 3) to determine
the sensitivity of MRA and CTA in patients with suspected renal artery
stenosis and 4) to test the hypothesis that the use of MRA and/or CTA
is cost-effective in patients with suspected renal artery stenosis.
“ENHANCED EVALUATION OF FETAL CNS WITH ULTRAFAST MRI”

The purpose of the study was to evaluate the utility of magnetic resonance
imaging (MRI) in diagnosing fetal abnormalities. This study compared the
accuracy of fast magnetic resonance imaging (MRI using the HASTE (Half
Fourier single-shot turbo-spin echo)) technique to ultrasound in the diagnosis
of CNS abnormalities.

C. Report of Current Research Activities:
Current foci of research interest include (1) new contrast agents for
evaluation of myocardial ischemia, (2) partially parallel imaging technology
for faster MRI, (3) imaging of pro-angiogenic therapy of ischemic heart
disease and (4) imaging of the mechanical properties of the lungs using
radiofrequency tagging methods. I am the PI collaborating with EagleVision
Pharmaceuticals on project (1), have two NIH-funded grants supporting
project (2), was the PI with Chiron Corporation for project (3), and am
an inventor of the method for project (4), in collaboration with Dr. Qun
Chen, physicist within the ENH MRI research group.

As Associate Radiologist-in-Chief for Academic Affairs at Beth Israel
Deaconess Medical Center, I helped developed and implement policies affecting
academic issues such as research and training and report directly to the
Radiologist-in-Chief. For instance, I chaired the Academic Time committee
which was charged with developing coherent policies for allocation of
academic and conference time.
As President of the ISMRM, I had primary administrative responsibility
for all matters affecting the leading international scientific society
involved with the biomedical applications of magnetic resonance.
As Chairman of the Department of Radiology at Evanston Northwestern Healthcare,
I have primary responsibility for management of all radiology-related
